To remain effective in today's operating environment, Army Aviation crewmembers must be able to operate sophisticated aircraft and weapons systems for extended periods in challenging conditions, including austere environments and adverse weather. However, the physiological effects of operating in such conditions can significantly impact aircrew performance and jeopardize mission success if not properly addressed through training and awareness of aeromedical factors.

Spatial disorientation (SD) is a leading cause of aircraft mishaps, and TC 3-04.1-6 provides updated guidance on mitigating this risk. By incorporating lessons learned from recent military conflicts and contingency operations, this publication offers the most relevant training methods for recognizing and managing SD. Throughout their careers, Army aviators have been trained to respond instinctively to certain situations, such as inadvertent instrument meteorological conditions and go-arounds. However, there is a need to improve training in articulating and managing potential spatial disorientation situations.

A key concept in addressing SD is the term "uncaged," which is defined as a state in which sensory perceptions of aircrew members become compromised as a result of environmental or physiological factors, leading to the onset of spatial disorientation. To mitigate this risk, crewmembers must consider several factors before every flight including anticipation, avoidance, communication, and countermeasures. This involves understanding risk factors, employing good aircrew coordination, assessing the geographical environment, avoiding hazardous weather conditions, and maintaining instrument proficiency. In the event of suspected SD, crewmembers should trust their instruments and ignore conflicting sensory inputs. In two-pilot aircraft, the standard procedure is to announce "uncaged" and transfer control to the other pilot.
