Failure Modes & Effects Analysis (FMEA) is a risk management analysis tool that can be used to manage risks caused by failure modes. Management of risks is normally an ongoing process that needs to be maintained over the product lifecycle. ‘Management’ of risks is an appropriate term in the context of dealing with risks that arise in design processes, products and manufacturing processes. Some risks will require reduction in their occurrence or overall risk profile. Sometimes, the severities of failure modes cannot be removed of reduced, should the failure mode occur. Therefore, if serious or severe consequences may occur, there should be a case that the benefit of the product or service has a very low likelihood of the risk occurring and that benefit outweighs the risk.
